Seeking an accomplished Licensed Practical Nurse (LPN) with a passion for home health care, this contract opportunity focuses on providing comprehensive, high-quality nursing support to geriatric patients in their residences. Schedule:
- Contract position with potential for extension
Key Qualifications:
- Current, active LPN license
- At least one year of recent, hands-on home health experience
- Demonstrated proficiency using HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment and documentation abilities
- Reliable transportation for regular home visits
- Strong organizational skills and the ability to independently manage a caseload
Primary Responsibilities:
- Deliver skilled nursing services to patients based on individualized care plans
- Monitor patient progress and update care plans in collaboration with interdisciplinary teams
- Accurately administer medications and conduct state-approved treatments
- Educate patients and their families on care techniques, disease prevention, and overall health management
- Maintain precise, thorough records using the HCHB electronic medical records system
- Coordinate care seamlessly with RNs, physical and occupational therapists, and other professionals to ensure optimal patient outcomes
