Schedule/Hours:

Required Qualifications:
- Active LPN license
- At least one year of recent home health experience
- Proficiency with HomeCare HomeBase (HCHB) EMR system
- Strong clinical assessment and documentation capabilities
- Dependable transportation for traveling to patient homes
- Ability to work independently and efficiently manage a caseload
Key Responsibilities:
- Deliver skilled nursing care in the home according to individualized care plans
- Monitor and track patient progress, updating care plans as necessary
- Administer medications and perform clinical treatments within state guidelines
- Educate patients and family members on disease prevention and care management
- Maintain detailed patient records utilizing HCHB EMR
