Key Qualifications:

  • Current, unencumbered LPN license
  • At least one year of recent home health experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR
  • Demonstrated clinical assessment and documentation abilities
  • Reliable transportation to conduct home visits
  • Comfort working independently and effectively managing a caseload

Primary Responsibilities:

  • Provide skilled nursing care according to individualized care plans in patients homes
  • Accurately monitor, document, and report patient progress
  • Administer medications and perform treatments in accordance with state regulations
  • Educate patients and families on care management strategies and disease prevention
  • Maintain comprehensive and timely documentation in HCHB
  • Collaborate with registered nurses, therapists, and other healthcare team members to optimize patient outcomes
