Job Description

Deliver high-quality nursing care directly to patients in their homes, focusing on geriatric populations as part of a contract home health assignment. This role requires experienced and compassionate Licensed Practical Nurses (LPNs) adept at managing individualized care and collaborating within an interdisciplinary healthcare team.

Schedule:

  • Contract-based role; scheduling details will be discussed during the interview process

Qualifications:

  • Active LPN license in good standing
  • At least one year of recent home health experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR
  • Strong clinical assessment and documentation abilities
  • Reliable transportation for patient home visits
  • Capacity to work independently and efficiently manage a caseload

Primary Responsibilities:

  • Deliver skilled nursing care in home settings in alignment with individualized care plans
  • Monitor patient conditions and document progress, revising care plans as appropriate
  • Administer prescribed medications and perform treatments per state guidelines
  • Educate patients and families on care management strategies and disease prevention
  • Accurately maintain records using the HomeCare HomeBase (HCHB) EMR system
  • Collaborate with RNs, therapists, and other healthcare professionals to provide holistic patient care
