Deliver high-quality nursing care directly to patients in their homes, focusing on geriatric populations as part of a contract home health assignment. This role requires experienced and compassionate Licensed Practical Nurses (LPNs) adept at managing individualized care and collaborating within an interdisciplinary healthcare team.
Schedule:
- Contract-based role; scheduling details will be discussed during the interview process
Qualifications:
- Active LPN license in good standing
- At least one year of recent home health experience
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment and documentation abilities
- Reliable transportation for patient home visits
- Capacity to work independently and efficiently manage a caseload
Primary Responsibilities:
- Deliver skilled nursing care in home settings in alignment with individualized care plans
- Monitor patient conditions and document progress, revising care plans as appropriate
- Administer prescribed medications and perform treatments per state guidelines
- Educate patients and families on care management strategies and disease prevention
- Accurately maintain records using the HomeCare HomeBase (HCHB) EMR system
- Collaborate with RNs, therapists, and other healthcare professionals to provide holistic patient care
