Job Description
Provide compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) within a dynamic field-based role. This contract position involves delivering skilled nursing visits to patients in their homes throughout the Phoenix, AZ area, promoting recovery, independence, and education.
Key Responsibilities:
- Assess, plan, and implement individualized care for home health patients during skilled nursing visits
- Administer medications, perform wound care, manage IV therapy, and execute clinical interventions as prescribed
- Monitor patient conditions; promptly communicate changes to RN Case Manager or physicians
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Document all care and communication accurately using the Home Care Home Base (HCHB) system
- Collaborate with interdisciplinary teams to coordinate care and ensure regulatory compliance
- Maintain infection control and patient safety standards in the home environment
- Participate in case conferences, quality improvement efforts, and continuing education
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year home health, hospice, or community-based care experience
- Proficient in Home Care Home Base (HCHB) documentation system
- Valid drivers license, reliable vehicle, and current insurance required
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, excellent communication, and ability to work independently
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Patient-centered approach with strong interpersonal skills
Schedule & Location:
- Full-time contract, Monday through Friday, 8:00 AM 5:00 PM, with occasional schedule flexibility
- Approximately 35 patient visits per day within an assigned territory in Phoenix, AZ
- Reliable transportation required; mileage reimbursement typically provided
