Job Description

Bring your compassionate nursing expertise to a rewarding role caring for patients in their homes. We are looking for a skilled and dedicated Licensed Practical Nurse (LPN) with home health experience to provide high-quality, patient-centered care within a field-based setting. This position emphasizes supporting patient independence, delivering essential education, and fostering recovery, while maintaining excellent communication with patients, families, and the broader healthcare team.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

  • Conduct skilled nursing visits, assessing and implementing personalized care plans for home health patients
  • Administer medications, perform wound care, manage IV therapy, and carry out clinical interventions per provider orders
  • Monitor patient conditions, promptly communicating significant changes to the RN Case Manager or physician
  • Educate patients and caregivers about disease management, medications, and self-care practices
  • Maintain meticulous documentation in Home Care Home Base (HCHB)
  • Coordinate with the interdisciplinary team to ensure continuity of care and compliance with care plans
  • Uphold infection control protocols and prioritize patient safety at all times
  • Participate in case conferences, quality improvement initiatives, and ongoing training

Qualifications:

  • Active, unrestricted LPN license
  • At least one year of recent experience in home health, hospice, or a similar care setting
  • Proficiency with Home Care Home Base (HCHB) required
  • Valid drivers license, reliable insured vehicle, and willingness to travel within the assigned territory
  • Current Basic Life Support (BLS) certification
  • Effective clinical judgment, strong communication, and time management skills
  • Ability to work independently while managing a variable caseload

Preferred Experience:

  • Medicare-certified home health background
  • Strong understanding of compliance and documentation requirements
