Job Description
Deliver skilled nursing care to patients in their homes as a Licensed Practical Nurse (LPN) specializing in home health. This full-time contract role involves managing a caseload of patients within an assigned territory, focusing on patient independence, education, and recovery. You will work closely with patients, families, and healthcare professionals to provide individualized, high-quality nursing care while adhering to home health compliance standards.
Key Responsibilities:
- Perform skilled nursing visits including medication administration, wound care, and IV therapy as ordered
- Assess patient conditions, develop and implement care plans aligned with regulatory requirements
- Monitor patient progress and report changes to RN Case Managers or physicians
- Educate patients and caregivers on disease management and self-care techniques
- Maintain accurate documentation using Home Care Home Base (HCHB) software
- Ensure infection control and patient safety protocols are followed
- Participate in team meetings, quality improvement, and training activities
Qualifications:
- Active and unrestricted LPN license in the state of Georgia
- Minimum one year of home health, hospice, or community care experience
- Proficiency with Home Care Home Base (HCHB) documentation system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Strong clinical judgment, time management, and effective communication skills Preferred:
- Experience with Medicare-certified home health agencies and compliance standards
- Excellent interpersonal skills with a patient-focused approach
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility to meet patient needs
- Approximate workload: 2532 points weekly (35 patient visits per day)
