Job Description
A Home Health Licensed Practical Nurse (LPN) opportunity is available to provide compassionate, patient-centered nursing care in a home-based setting. This role focuses on promoting patient independence and recovery while maintaining effective communication with patients, families, and the interdisciplinary healthcare team.
Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans for home health patients
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions as ordered
- Monitor changes in patient conditions and promptly report findings to case managers or physicians
- Educate patients and caregivers on disease processes, medication management, and self-care techniques
- Accurately document assessments, care provided, and communications using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to coordinate care and ensure compliance with care plans
- Maintain infection control standards and ensure patient safety throughout visits
- Participate in case conferences, quality improvement activities, and required training
Qualifications:
- Active and unrestricted LPN license in the applicable state
- Minimum one year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) software
- Valid driver's license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication abilities
- Ability to work independently in a field-based environment managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-centered approach
Schedule & Location:
- Full-time contract position
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility based on patient needs
- Work is field-based within assigned territories; reliable transportation required (mileage reimbursement typically provided)
- Caseload includes approximately 35 patient visits per day (2532 points weekly)
