Job Description

Providing compassionate, skilled nursing care in patients' homes, this full-time contract opportunity offers a rewarding role for an experienced Licensed Practical Nurse (LPN) specializing in home health. Operating in a field-based setting within an assigned territory, the nurse will manage a caseload of 35 patient visits per day, ensuring individualized care plans are effectively implemented and documented in Home Care Home Base (HCHB).

Responsibilities:

  • Conduct skilled nursing assessments and implement care plans tailored to each patient's needs.
  • Administer medications, wound care, IV therapy, and other clinical interventions as prescribed.
  • Monitor patient conditions, promptly communicating changes to RN Case Managers or physicians.
  • Educate patients and caregivers about disease management, medications, and self-care strategies.
  • Maintain accurate documentation using the HCHB electronic charting system.
  • Collaborate with the interdisciplinary healthcare team to coordinate care and comply with regulatory standards.
  • Uphold infection control protocols and maintain patient safety during all visits.
  • Engage in case conferences, quality improvement projects, and mandatory training sessions.

Qualifications and Desired Experience:

  • Active and unrestricted LPN license in the practicing state.
  • Minimum one year of home health, hospice, or community-based nursing experience.
  • Proficiency with Home Care Home Base (HCHB) electronic medical records system.
  • Valid driver's license, reliable insured vehicle, and ability to travel within the assigned area.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical judgment, excellent communication, and effective time management skills.
  • Ability to work independently in a mobile, patient-focused role.
  • Preferred: Experience with Medicare-certified home health agencies and thorough knowledge of home health compliance.

Schedule:

  • Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility to meet patient needs.
