Job Description

Schedule/Hours:

  • Contract-based position with potential for extension

Qualifications:

  • Current, active LPN license
  • At least one year of recent home health nursing experience
  • Proficiency in HomeCare HomeBase (HCHB) EMR system
  • Strong clinical assessment and documentation abilities
  • Reliable transportation for conducting patient home visits
  • Self-motivated with the capacity to manage a caseload independently

Responsibilities:

  • Deliver skilled nursing services to patients in their homes, adhering to individualized care plans
  • Monitor patient progress and update care plans in collaboration with the healthcare team
  • Administer medications, perform treatments, and follow state guidelines for nursing care
  • Provide education to patients and their families on managing care and preventing disease
  • Document all care and communications accurately in the HCHB electronic medical record system
  • Coordinate seamlessly with RNs, therapists, and other healthcare providers to ensure fully integrated patient care
