Schedule/Hours:
- Contract-based position with potential for extension
Qualifications:
- Current, active LPN license
- At least one year of recent experience in a home health setting
- Proficient in HomeCare HomeBase (HCHB) EMR for documentation and care coordination
- Solid clinical assessment, communication, and documentation skills
- Reliable transportation for travel between patient homes
- Demonstrated ability to work independently and maintain organizational efficiency with a dedicated caseload
Responsibilities:
- Deliver skilled nursing care to patients following established care plans
- Accurately monitor, assess, and document patient progress, updating care plans as appropriate
- Safely administer medications and perform procedures within state licensure guidelines
- Educate patients and their loved ones on disease management, preventive care, and safe practices
- Maintain comprehensive, accurate records using HomeCare HomeBase (HCHB) EMR system
- Collaborate closely with RNs, physical therapists, and other healthcare providers to deliver coordinated, patient-centered care
