Schedule/Hours:
- Contract-based position with potential for extension
Qualifications:
- Current, active LPN license
- At least one year of recent home health nursing experience
- Proficiency in HomeCare HomeBase (HCHB) EMR system
- Strong clinical assessment and documentation abilities
- Reliable transportation for conducting patient home visits
- Self-motivated with the capacity to manage a caseload independently
Responsibilities:
- Deliver skilled nursing services to patients in their homes, adhering to individualized care plans
- Monitor patient progress and update care plans in collaboration with the healthcare team
- Administer medications, perform treatments, and follow state guidelines for nursing care
- Provide education to patients and their families on managing care and preventing disease
- Document all care and communications accurately in the HCHB electronic medical record system
- Coordinate seamlessly with RNs, therapists, and other healthcare providers to ensure fully integrated patient care
