Schedule/Hours:
- Flexible schedule based on patient needs and case assignments
Qualifications:
- Active LPN license
- At least one year of recent home health experience
- Proficiency in HomeCare HomeBase (HCHB) EMR
- Skilled in clinical assessment and documentation
- Reliable transportation is required for home visits
- Ability to work autonomously and efficiently manage a caseload
Responsibilities:
- Deliver skilled nursing care to patients in their own homes following individualized care plans
- Monitor patient progress, updating care plans and documentation as necessary
- Administer prescribed medications and perform treatments under state regulations
- Educate patients and families on care management, disease prevention, and health maintenance
- Maintain thorough and accurate records with HomeCare HomeBase (HCHB) EMR
- Collaborate closely with registered nurses, therapists, and other health professionals for comprehensive patient care
