Job Description

Deliver compassionate, high-quality care as a Licensed Practical Nurse specializing in home health. This contract role centers on engaging directly with patients in their residences, fostering independence, and supporting recovery. Ideal candidates value strong communication, are adept at collaborative care planning, and are experienced in meeting regulatory and documentation standards via Home Care Home Base (HCHB).

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility to accommodate patient needs)
  • Full-time contract commitment
  • Manage 2532 points per week (about 35 patient visits daily)
  • Field-based setting with travel within your assigned area; reliable transportation and current drivers license required

Key Responsibilities:

  • Conduct skilled nursing assessments, implement and adapt individualized care plans
  • Administer medications, wound care, IV therapy, and other nursing interventions as prescribed
  • Monitor patient conditions, escalate concerns to Case Managers or physicians as needed
  • Empower patients and caregivers through education on health management and self-care
  • Maintain timely and accurate documentation in Home Care Home Base (HCHB)
  • Coordinate with interdisciplinary teams to ensure compliance and optimal patient outcomes
  • Follow infection control protocols, promoting patient safety at all times
  • Participate in team meetings, case conferences, and ongoing training

Required Qualifications:

  • Active, unrestricted LPN license for the state
  • Minimum 1 year experience in home health, hospice, or similar care setting
  • Proficiency in HCHB electronic documentation
  • Current BLS certification
  • Strong problem-solving, communication, and time management skills
  • Ability to manage a dynamic caseload independently

Preferred:

  • Previous work with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation standards
