Job Description

Provide compassionate, skilled nursing care as a Home Health Licensed Practical Nurse (LPN) in a field-based role serving patients within their homes. This contract position offers an opportunity to make a meaningful impact on patients health management and recovery through personalized care plans and ongoing patient education. You will manage your caseload independently, ensuring compliance with home health regulations while maintaining clear communication with patients, families, and the healthcare team.

Responsibilities:

  • Conduct skilled nursing visits to assess patient conditions and develop individualized care plans
  • Administer medications, perform wound care, and manage IV therapy as prescribed
  • Monitor patient status changes and report to the RN Case Manager or physician promptly
  • Educate patients and caregivers on disease management, medication adherence, and self-care techniques
  • Accurately document all nursing activities using Home Care Home Base (HCHB) system
  • Collaborate closely with the interdisciplinary healthcare team to coordinate care and meet compliance standards
  • Uphold infection control and patient safety protocols during all visits
  • Participate in case conferences, training, and quality improvement initiatives

Qualifications and Desired Experience:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of 1 year experience in home health, hospice, or community-based care
  • Proficiency with Home Care Home Base (HCHB) electronic charting system
  • Valid drivers license, reliable transportation, and current vehicle insurance
  • Current BLS (Basic Life Support) certification
  • Excellent clinical judgment, communication, and time-management skills
  • Ability to work autonomously in a field-based role managing a dynamic patient caseload

Preferred:

  • Experience within Medicare-certified home health agencies
  • Strong knowledge of home health compliance and documentation requirements
  • Patient-centered approach with outstanding interpersonal skills

Schedule:

  • Monday through Friday, 8:00 AM to 5:00 PM (some scheduling flexibility for patient needs)

Contract Details:

  • Full-time contract position
  • Approximately 35 patient visits per day (2532 points weekly workload)
  • Mileage reimbursement typically provided
