Job Description
Provide compassionate, skilled nursing care as a Home Health Licensed Practical Nurse (LPN) delivering patient-centered services in individual homes within an assigned territory. This full-time contract position involves managing a caseload of patients, promoting independence and recovery while maintaining close communication with patients, families, and the healthcare team.
Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement individualized care plans for home health patients.
- Administer medications, provide wound care, and manage IV therapy or other ordered clinical interventions.
- Monitor patient conditions, reporting changes promptly to the RN Case Manager or physician.
- Educate patients and caregivers on disease processes, medication adherence, and self-care techniques.
- Accurately document all assessments, nursing care, and communications using Home Care Home Base (HCHB).
- Collaborate with the interdisciplinary team to coordinate care and ensure compliance with regulatory standards.
- Uphold infection control and patient safety guidelines.
- Participate in case conferences, quality improvement efforts, and required training.
Qualifications and Experience:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 year experience in home health, hospice, or community-based care.
- Proficient with Home Care Home Base (HCHB) charting system.
- Valid drivers license, reliable insured vehicle, and availability for travel within the area.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, time management, and communication skills.
- Ability to work independently and manage a dynamic caseload.
Preferred:
- Experience with Medicare-certified home health agencies.
- Knowledge of home health compliance and documentation standards.
- Excellent interpersonal skills with a patient-focused approach.

Schedule:
