Qualifications:
- Active LPN license in good standing
- Minimum one year of recent home health nursing experience
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment and detailed documentation skills
- Reliable transportation for community-based visits
- Ability to work independently and manage a caseload efficiently
Key Responsibilities:
- Provide professional nursing care to patients in their homes, following individualized care plans
- Monitor patient progress and update care plans accordingly
- Administer medications and carry out treatments in compliance with state regulations and best practices
- Educate patients and families on disease prevention and ongoing care management
- Maintain thorough, accurate records and documentation via HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare professionals to deliver well-coordinated patient care
