Deliver compassionate, quality care as a Home Health Licensed Practical Nurse (LPN) providing vital support to patients within their homes. This contract position offers the opportunity to empower individuals by promoting independence, education, and recovery. As a field-based nurse, youll build meaningful relationships while driving positive health outcomes in collaboration with a skilled interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility for patient needs)
• Field-based: Serve patients in their homes across an assigned territory
• Manage a weekly workload of 2532 points (equivalent to approximately 35 patient visits per day)

Key Responsibilities:

• Perform skilled nursing visits, assess patient needs, and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy or other physician-directed interventions
• Track patient condition changes and communicate promptly with RN Case Manager or physician
• Educate patients and caregivers on their condition, medication routines, and self-care best practices
• Document all care and communication accurately using the Home Care Home Base (HCHB) system
• Maintain infection control standards and participate in collaborative case conferences and ongoing training

Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least 1 year of experience in home health, hospice, or community-based care
• Hands-on proficiency with Home Care Home Base (HCHB) required
• Current BLS (Basic Life Support) certification
• Valid drivers license, reliable vehicle, and up-to-date insurance
• Strong organization, time management, and clinical assessment abilities
• Ability to work independently in the field and adapt to a dynamic caseload

Preferred:

• Experience in Medicare-certified home health agencies
• Advanced knowledge of home health compliance and documentation standards
• Excellent patient-focused communication and collaboration skills
