Job Description

Provide expert nursing care as a Home Health Licensed Practical Nurse (LPN) delivering patient-centered services in home settings. This full-time contract position involves managing a caseload of patients living within an assigned territory, supporting recovery, promoting independence, and ensuring adherence to home health regulations.

Responsibilities include conducting skilled nursing visits to assess and implement individualized care plans, administering medications, managing wound care and IV therapy, monitoring patient conditions, and educating patients and caregivers. Accurate documentation using the Home Care Home Base (HCHB) system is essential, as is collaborating with the interdisciplinary team to align care with compliance standards. Maintaining infection control and patient safety is a critical part of the role, along with participating in case conferences and quality improvement initiatives.

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of experience in home health, hospice, or similar community-based care
  • Proficiency with Home Care Home Base (HCHB) documentation system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current Basic Life Support (BLS) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently managing a dynamic caseload in the field

Preferred:

  • Experience with Medicare-certified home health agencies
  • Thorough knowledge of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-focused care approach

Schedule & Setting:

  • Full-time contract, Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility
  • Field-based position with approximately 35 patient visits daily (2532 points weekly)
  • Reliable transportation required; mileage reimbursement typically provided
