Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in a field-based setting. This full-time contract role involves managing a caseload of home health patients, delivering skilled nursing interventions, and ensuring thorough documentation aligned with home health compliance standards.

Key Responsibilities:

• Conduct skilled nursing visits for patient assessment, care planning, medication administration, wound care, and clinical interventions as prescribed.
• Monitor patient conditions, communicate changes promptly to the RN Case Manager or physician.
• Educate patients and caregivers on medication management, disease processes, and self-care techniques.
• Maintain accurate documentation using the Home Care Home Base (HCHB) electronic charting system.
• Collaborate with the interdisciplinary healthcare team to coordinate patient care and uphold care plan compliance.
• Ensure adherence to infection control and patient safety standards.
• Participate in case conferences, quality improvement initiatives, and required training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum of 1 years experience in home health, hospice, or community-based care.
• Proficient in Home Care Home Base (HCHB).
• Valid drivers license, reliable insured vehicle required; mileage reimbursement typically provided.
• Current Basic Life Support (BLS) certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently managing a dynamic caseload in the field.

Preferred:

• Experience with Medicare-certified home health agencies.
• Deep understanding of home health compliance and documentation standards.
• Excellent interpersonal skills and a patient-centered approach.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility based on patient needs.
