Job Description
Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in a field-based setting. This full-time contract role involves managing a caseload of home health patients, delivering skilled nursing interventions, and ensuring thorough documentation aligned with home health compliance standards.
Key Responsibilities:
- Conduct skilled nursing visits for patient assessment, care planning, medication administration, wound care, and clinical interventions as prescribed.
- Monitor patient conditions, communicate changes promptly to the RN Case Manager or physician.
- Educate patients and caregivers on medication management, disease processes, and self-care techniques.
- Maintain accurate documentation using the Home Care Home Base (HCHB) electronic charting system.
- Collaborate with the interdisciplinary healthcare team to coordinate patient care and uphold care plan compliance.
- Ensure adherence to infection control and patient safety standards.
- Participate in case conferences, quality improvement initiatives, and required training.
Qualifications:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 years experience in home health, hospice, or community-based care.
- Proficient in Home Care Home Base (HCHB).
- Valid drivers license, reliable insured vehicle required; mileage reimbursement typically provided.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, time management, and communication skills.
- Ability to work independently managing a dynamic caseload in the field.
Preferred:
- Experience with Medicare-certified home health agencies.
- Deep understanding of home health compliance and documentation standards.
- Excellent interpersonal skills and a patient-centered approach.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility based on patient needs.
