A skilled and compassionate Licensed Practical Nurse (LPN) is needed for a field-based home health assignment. The role involves delivering high-quality nursing care to patients in their homes, focusing on patient education, promoting independence, and ensuring excellent communication with patients, families, and the interdisciplinary care team. This contract assignment offers consistent, weekday scheduling and requires strong documentation and compliance skills, particularly with Home Care Home Base (HCHB).

Schedule & Work Details:

• Monday through Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
• 2532 points per week, averaging 35 patient visits per day
• Field-based setting, visiting patients within assigned areas
• Reliable personal transportation required; mileage reimbursement typically available

Key Responsibilities:

• Perform skilled nursing visits, assessments, and individualized care planning
• Administer medications, conduct wound care, and manage IV therapy as ordered
• Observe patient conditions and report changes to the RN Case Manager or physician
• Provide education to patients and caregivers on health management and self-care
• Accurately document care and communication using HCHB
• Collaborate with interdisciplinary team members for coordinated patient care
• Maintain infection control and ensure patient safety
• Attend case conferences and participate in training and quality initiatives

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum 1 year of recent home health or community-based care experience
• Proficiency with Home Care Home Base (HCHB) required
• Valid drivers license, current vehicle insurance, and reliable transportation
• Current BLS certification
• Strong clinical, communication, and time management skills
• Ability to work independently within the field

Preferred:

• Experience with Medicare-certified home health
• Familiarity with home health compliance and documentation standards
