Job Description
Deliver personalized, patient-focused care in the community as a Licensed Practical Nurse specializing in home health services. This contract role calls for compassion, clinical expertise, and the flexibility to meet the unique needs of individuals in their homes. Youll manage a dedicated caseload while coordinating with a dynamic healthcare team to promote optimal healing and independence for every patient.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility to accommodate patient needs
Desired Experience & Qualifications:
- Active, unrestricted LPN license valid in the state of practice
- At least 1 year of recent experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) electronic charting
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable transportation, and insurance
- Demonstrated clinical judgement, effective communication, and time management skills
- Ability to work independently, multitask, and adapt to changing situations
Preferred Qualifications:
- Experience in Medicare-certified home health settings
- In-depth knowledge of home health compliance requirements
- Exceptional interpersonal skills and a patient-centered mindset
Key Responsibilities:
- Conduct thorough nursing assessments and implement individualized care plans during in-home visits
- Provide skilled interventions, such as medication administration, wound care, and IV management
- Monitor and document patient progress in compliance with care standards and regulations
- Educate patients and caregivers on managing diseases, medications, and home safety
- Collaborate with the RN Case Manager and interdisciplinary team
- Uphold infection control best practices and contribute to ongoing quality improvement initiatives
