Job Description
Embark on a rewarding career as an experienced and compassionate Licensed Practical Nurse specializing in home health care. This contract assignment allows you to deliver high-quality, patient-centered nursing services directly in patients homes, while working closely with families and the interdisciplinary healthcare team to foster patient independence and recovery.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or similar community-based care setting
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, time management, and communication abilities
- Ability to manage a dynamic caseload independently in the field
Preferred Attributes:
- Previous work with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation requirements
- Excellent interpersonal skills and a patient-focused mindset
Core Responsibilities:
- Complete skilled nursing visits and develop individualized care plans for home health patients
- Administer prescribed medications, perform wound care, and manage IV therapy or other clinical interventions
- Closely monitor patient conditions and promptly communicate findings to supervising RNs or physicians
- Educate patients and caregivers on disease management, self-care, and medication
- Consistently document all care and communication using HCHB system
- Collaborate across the healthcare team to ensure adherence to care plans and promote positive outcomes
- Adhere to infection control procedures and uphold patient safety standards
- Attend case conferences, quality improvement activities, and required trainings
