Expertise in home health, coupled with proficiency in HomeCare HomeBase (HCHB) EMR, is essential for ensuring high standards of care and accurate documentation throughout the assignment.
Schedule/Hours:
- Contract position; duration to be discussed with candidates during the interview process.
Qualifications:
- Current, active LPN license
- Minimum of one year of recent home health clinical experience
- Proficient in HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment and precise documentation abilities
- Reliable transportation for visiting patients homes
- Ability to work independently while efficiently managing a caseload
Core Responsibilities:
- Delivering skilled nursing care to patients at home, following individualized care plans
- Monitoring patient progress and documenting updates or changes in condition
- Administering medications and treatments in alignment with state regulations
- Educating patients and families on care management, medication adherence, and preventive health practices
- Maintaining thorough, up-to-date records using HCHB EMR
- Coordinating care with RNs, therapists, and other medical professionals to ensure comprehensive treatment
