We are seeking an experienced Home Health Licensed Practical Nurse (LPN) ready to make a meaningful impact through individualized care for patients in their homes. This contract role is ideal for a nurse who thrives in a field-based environment, enjoys promoting independence, and is dedicated to exceptional clinical standards.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (some flexibility for patient needs)
• 2532 points per week (approximately 35 patient visits per day)
• Field-based within an assigned territory

Key Responsibilities:

• Complete skilled nursing assessments and implement customized care plans for home health patients
• Administer medications, wound care, and manage clinical interventions (including IV therapy when ordered)
• Observe and report changes in patient health promptly to RN Case Manager or physician
• Educate patients and caregivers on disease management and self-care best practices
• Maintain thorough charting and documentation via Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary care team for coordinated care
• Uphold infection prevention measures and ensure a safe environment
• Participate in case conferences, ongoing training, and quality improvement

Qualifications:

• Active, unrestricted LPN license
• At least 1 year of home health, hospice, or similar experience
• Experienced with Home Care Home Base (HCHB)
• Current BLS certification
• Valid drivers license, reliable transportation, and current insurance
• Excellent clinical judgment, organization, and communication skills
• Comfortable managing care independently in the field

Preferred:

• Background with Medicare-certified home health agencies
• Solid understanding of home health compliance and documentation

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
