Deliver essential patient care in a rewarding home health environment, supporting geriatric clients in their homes. This contract position is ideal for a Licensed Practical Nurse (LPN) with home health experience who is skilled in using HomeCare HomeBase (HCHB) EMR and passionate about delivering high-quality, compassionate care.
Key Qualifications:
- Active LPN license required
- At least one year of recent experience in home health
- Proficient with HomeCare HomeBase (HCHB) electronic medical records
- Excellent clinical assessment, communication, and documentation abilities
- Reliable transportation for patient visits
- Demonstrated ability to work independently and efficiently manage a caseload
Schedule/Hours:
- Contract assignment; duration discussed during the interview process
Responsibilities:
- Provide skilled nursing services to adult and geriatric patients in their homes, following individualized care plans
- Monitor, assess, and document patient progress; update care plans as needed
- Administer medications and perform prescribed treatments in accordance with state regulations
- Educate patients and their families on ongoing care needs, disease prevention, and health management
- Maintain accurate, timely records utilizing HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare professionals to ensure comprehensive care solutions
