Job Description

Deliver high-quality, patient-focused care as a Home Health Licensed Practical Nurse (LPN). This contract, full-time opportunity involves providing nursing services to patients in their homes, with an emphasis on promoting independence, education, and recovery. The successful candidate will manage a caseload efficiently, ensuring compliance with all relevant standards while maintaining proactive communication with patients, families, and the broader healthcare team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility may be required for patient needs

Key Qualifications:

  • Active, unrestricted LPN license (state of practice)
  • At least 1 year of experience in home health, hospice, or a related setting
  • Proficiency with Home Care Home Base (HCHB) charting system
  • Valid drivers license and reliable vehicle with current insurance
  • Current BLS certification
  • Strong clinical judgment, time management, and communication abilities
  • Capacity to work independently and manage a field-based caseload

Preferred Experience:

  • Background with Medicare-certified home health agencies
  • Deep understanding of home health compliance and documentation standards
  • Patient-focused approach and excellent interpersonal skills

Primary Responsibilities:

  • Perform skilled nursing visits to assess, plan, and deliver individualized care
  • Administer medications, wound care, IV therapy, and other ordered interventions
  • Monitor and document patient status using HCHB, reporting changes promptly
  • Educate patients and caregivers on diseases, medications, and self-care
  • Coordinate care and collaborate with the multidisciplinary team
  • Attend case conferences and participate in quality improvement activities
  • Ensure adherence to infection control and patient safety protocols
