Job Description

Deliver compassionate, patient-focused care as a Home Health Licensed Practical Nurse (LPN) providing skilled nursing services in patients homes. This field-based role requires clinical expertise, strong communication, and the ability to manage a caseload independently while ensuring adherence to care plans and regulatory standards.

Role Responsibilities:

  • Conduct thorough nursing assessments and develop individualized care plans for home health patients
  • Administer medications, provide wound care, and manage IV therapy or other clinical treatments as prescribed
  • Monitor patient condition changes and report promptly to RNs or physicians
  • Educate patients and caregivers on disease management, medication adherence, and self-care techniques
  • Complete accurate documentation using Home Care Home Base (HCHB) software
  • Collaborate with interdisciplinary healthcare teams to coordinate care and maintain compliance
  • Uphold infection control practices and ensure patient safety during visits
  • Participate in case conferences, continuous quality improvement initiatives, and mandatory trainings

Qualifications & Experience:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of one year experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB) charting system
  • Valid drivers license, reliable vehicle with current insurance
  • Current BLS certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently in a home setting managing multiple patients

Preferred:

  • Experience with Medicare-certified home health agencies
  • Deep understanding of home health compliance and documentation standards
  • Excellent interpersonal skills and a patient-centered approach to care

Schedule & Contract Details:

  • Full-time contract position
  • MondayFriday, 8:00 AM 5:00 PM with some flexibility
  • Caseload averages 2532 points weekly, equating to about 35 patient visits daily
