Job Description
Deliver compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) serving individuals in their residences throughout Myrtle Creek, OR. This full-time contract position offers the opportunity to work independently in the field, managing a caseload of patients while fostering recovery, education, and independence.
Key Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement individualized care within patients homes
- Administer medications, provide wound care, and conduct clinical interventions such as IV therapy
- Monitor patient conditions, reporting changes promptly to RN Case Managers or physicians
- Educate patients and caregivers on disease management and self-care techniques
- Maintain accurate and timely documentation using Home Care Home Base (HCHB) software
- Collaborate with interdisciplinary healthcare teams to ensure coordinated, compliant patient care
- Uphold infection control standards and prioritize patient safety
- Participate in case conferences, quality improvement activities, and mandatory trainings
Qualifications:
- Active, unrestricted LPN license in good standing for the state of practice
- Minimum one year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable insured vehicle, and ability to travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, effective time management, and excellent communication skills
- Ability to work independently managing a diverse caseload in a field environment
Preferred Experience:
- Familiarity with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Strong interpersonal skills with a patient-centered approach to care
Schedule and Workload:
- Monday through Friday, 8:00 AM to 5:00 PM with occasional schedule flexibility to meet patient needs
- Approximately 2532 points weekly, equating to 35 patient visits per day
