Job Description
Deliver essential care and support to patients as a Home Health Licensed Practical Nurse (LPN) in a dynamic and rewarding field-based role. This opportunity empowers you to provide direct nursing care in patients homes, focusing on individualized treatment, education, and patient independence. With full-time, contract hours from Monday through Friday, 8:00 AM to 5:00 PM (with occasional flexibility), youll manage a caseload equivalent to 2532 points per weekamounting to about 35 home visits per day within your assigned territory.
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or similar community-based roles
- Proficiency with Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current auto insurance
- Current BLS (Basic Life Support) certification
- Excellent clinical, organizational, and communication skills
- Ability to work autonomously and manage a varied, dynamic caseload
Preferred Experience:
- Background in Medicare-certified home health agencies
- Solid grasp of home health compliance and documentation standards
- Patient-focused attitude and strong interpersonal skills
Role Responsibilities:
- Conduct patient assessments, develop and implement care plans, and perform skilled nursing interventions (medications, wound care, IV therapy)
- Monitor patient conditions and report changes promptly
- Educate patients and families on disease management, medications, and self-care
- Collaborate with interdisciplinary teams to ensure seamless care
- Maintain thorough documentation in Home Care Home Base (HCHB)
- Attend case conferences, trainings, and quality improvement initiatives
- Adhere to infection control protocols for the safety of all
