Deliver expert nursing care directly in patients homes as a Licensed Practical Nurse (LPN) specializing in home health. This full-time contract position is based in Mount Washington, KY, offering a rewarding opportunity to impact the lives of community members through compassionate, patient-centered care.
Responsibilities include conducting skilled nursing visits to assess and implement individualized care plans, administering medications, performing wound care, and managing clinical interventions such as IV therapy. You will monitor patient conditions, provide education on disease management and self-care, and maintain clear, accurate documentation using the Home Care Home Base (HCHB) system. Collaboration with interdisciplinary teams ensures coordinated, high-quality care while complying with home health standards. Maintaining patient safety and infection control is paramount in this field-based role.
Qualifications & Desired Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of home health, hospice, or community-based nursing experience
- Proficiency with Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently managing a caseload in the field
- Preferred: Experience in Medicare-certified home health agencies and strong knowledge of compliance standards
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility to meet patient needs)
- Approximately 35 patient visits daily (2532 points weekly)
