Job Description
Deliver skilled, compassionate nursing care as a Home Health Licensed Practical Nurse (LPN) providing patient-centered services in individuals homes across Mount Sterling, KY. This full-time contract role offers the chance to manage a rewarding caseload while promoting patient independence, recovery, and education in a supportive field-based environment.
Key Responsibilities:
- Conduct comprehensive skilled nursing visits to assess, plan, and implement personalized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as prescribed
- Monitor patient conditions, promptly reporting changes to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Maintain accurate documentation of all assessments and care provided using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to coordinate care and ensure regulatory compliance
- Uphold infection control standards and prioritize patient safety during all visits
- Participate in case conferences, quality initiatives, and ongoing training
Qualifications:
- Active, unrestricted LPN license in the practicing state
- Minimum of 1 year experience in home health, hospice, or community-based care
- Proficiency in Home Care Home Base (HCHB) software
- Valid drivers license, reliable vehicle, and appropriate insurance
- Current BLS certification
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently managing a dynamic caseload in the field
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused approach
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility
