Embark on a rewarding opportunity as a Home Health Licensed Practical Nurse (LPN), providing essential field-based nursing care to patients in their homes. The focus of this contract position is on patient-centered care, fostering independence, and facilitating recovery through skilled assessments, education, and collaboration with families and healthcare professionals.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs

Key Responsibilities:

• Deliver skilled nursing visits to assess, create, and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy as ordered
• Monitor and promptly communicate changes in patient conditions to the supervising RN or physician
• Provide education to patients and caregivers on disease management, medications, and home care techniques
• Accurately document assessments, interventions, and communications within Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members to ensure alignment with patient care plans
• Maintain infection control protocols and ensure patient safety at all times
• Participate in case conferences, trainings, and quality improvement initiatives

Required Qualifications:

• Current, unrestricted LPN license in the state of practice
• Minimum 1 year of home health, hospice, or similar community-based nursing experience
• Proficient using Home Care Home Base (HCHB)
• Valid drivers license, reliable transportation, and current auto insurance
• Active BLS certification
• Strong time management, clinical decision-making, and communication skills
• Ability to effectively manage a dynamic field caseload independently

Preferred Experience:

• Previous work in Medicare-certified home health settings
• In-depth knowledge of home health documentation and compliance
