Deliver compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in a field-based role within Mount Lemmon, AZ. This full-time contract position involves managing a caseload of patients, providing skilled nursing visits, and collaborating closely with interdisciplinary teams to promote patient independence and recovery in their homes.

Key Responsibilities:

• Conduct thorough nursing assessments and implement individualized care plans.
• Administer medications, perform wound care, and manage clinical interventions such as IV therapy.
• Monitor patient conditions and communicate changes promptly to supervising healthcare providers.
• Educate patients and caregivers on disease management, medication, and self-care techniques.
• Accurately document all visits and care activities using Home Care Home Base (HCHB).
• Ensure compliance with infection control standards and patient safety protocols.
• Participate in case conferences, quality improvement, and ongoing training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum one year of experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB) electronic charting system.
• Valid drivers license, reliable vehicle, and current auto insurance.
• Current Basic Life Support (BLS) certification.
• Excellent clinical judgment, time management, and communication skills.
• Ability to work independently within a field-based setting and handle a dynamic patient caseload.

Preferred:

• Experience with Medicare-certified home health agencies.
• Strong knowledge of home health compliance and documentation standards.
• Patient-centered care approach with excellent interpersonal skills.

Schedule & Workload:

• Monday through Friday, 8:00 AM to 5:00 PM, with occasional schedule flexibility.
• Manage approximately 35 patient visits per day (2532 points weekly).
