Job Description
Embark on a rewarding opportunity as a Home Health Licensed Practical Nurse (LPN), providing essential field-based nursing care to patients in their homes. The focus of this contract position is on patient-centered care, fostering independence, and facilitating recovery through skilled assessments, education, and collaboration with families and healthcare professionals.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Responsibilities:
- Deliver skilled nursing visits to assess, create, and implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy as ordered
- Monitor and promptly communicate changes in patient conditions to the supervising RN or physician
- Provide education to patients and caregivers on disease management, medications, and home care techniques
- Accurately document assessments, interventions, and communications within Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members to ensure alignment with patient care plans
- Maintain infection control protocols and ensure patient safety at all times
- Participate in case conferences, trainings, and quality improvement initiatives
Required Qualifications:
- Current, unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or similar community-based nursing experience
- Proficient using Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current auto insurance
- Active BLS certification
- Strong time management, clinical decision-making, and communication skills
- Ability to effectively manage a dynamic field caseload independently
Preferred Experience:
- Previous work in Medicare-certified home health settings
- In-depth knowledge of home health documentation and compliance
