- Minimum of one year recent experience in home health

- Solid clinical assessment and documentation skills
- Reliable transportation for in-home patient visits
- Self-motivated with the ability to manage an independent caseload
Responsibilities:
- Deliver skilled, individualized nursing care according to established care plans
- Administer medications and perform treatments within LPN scope of practice
- Monitor and document patient progress, updating care plans as needed
