Job Description
Seeking a skilled and dedicated Licensed Practical Nurse (LPN) with expertise in home health care to provide patient-centered nursing services in a field-based role. This opportunity involves caring for individuals in their homes, promoting independence, and supporting recovery while maintaining compliance and high clinical standards.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility to accommodate patient needs
- Field-based with 2532 points per week (approximately 35 patient visits per day)
Key Responsibilities:
- Conduct skilled nursing visits, assess patient needs, and implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy or other clinical treatments as directed
- Monitor patient conditions and report significant changes promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication protocols, and self-care practices
- Accurately document assessments and care delivered using Home Care Home Base (HCHB) system
- Coordinate with the interdisciplinary healthcare team to ensure seamless patient care and regulatory compliance
- Uphold infection control best practices and patient safety standards
- Participate in case conferences and ongoing training sessions
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or community-based care setting
- Proficient with Home Care Home Base (HCHB) documentation and charting
- Valid drivers license, reliable transportation, and current insurance
- BLS (Basic Life Support) certification
- Ability to manage a variable caseload independently and efficiently
- Strong clinical judgment, organization, and communication skills
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of compliance and documentation requirements in home health
- Patient-focused approach and strong interpersonal skills
