Job Description

Deliver specialized care in the community by joining a skilled home health nursing team. Seeking an experienced and compassionate Licensed Practical Nurse (LPN) who excels in patient-centered care, documentation, and clinical excellence within home settings.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required for patient needs

Responsibilities:

  • Conduct skilled nursing visits in patient homesmanaging assessments, developing care plans, and performing interventions such as wound care, medication administration, and IV therapy.
  • Monitor patient conditions closely and promptly communicate changes to the RN Case Manager or physician.
  • Educate patients and caregivers on disease management, medication routines, and independent self-care.
  • Utilize the Home Care Home Base (HCHB) system for comprehensive and accurate documentation.
  • Coordinate with interdisciplinary healthcare professionals to optimize patient care and ensure compliance with current plans.
  • Maintain rigorous infection control standards and uphold the highest standards of patient safety.
  • Engage in case conferences, ongoing training, and quality improvement initiatives.

Qualifications:

  • Active, unrestricted LPN license in the relevant state
  • At least one year of home health, hospice, or similar experience
  • Proficiency with Home Care Home Base (HCHB) charting system
  • Current BLS (Basic Life Support) certification
  • Valid drivers license and access to an insured, reliable vehicle
  • Strong clinical judgment, independence, and time management skills
  • Comfortable managing a dynamic caseload in a field-based setting

Preferred:

  • Experience in Medicare-certified home health agency environments
  • Solid grasp of home health compliance and documentation
  • Exceptional interpersonal and patient-focused communication abilities
