Job Description

Seeking a skilled and compassionate Home Health Licensed Practical Nurse (LPN) to provide high-quality, patient-focused care in a field-based role. As part of the home health team, youll promote patient independence, education, and recovery while partnering closely with patients, families, and care teams. This contract position requires expertise in Home Care Home Base (HCHB) and a strong understanding of home health compliance requirements.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

  • Conduct skilled nursing visits to assess, plan, and implement individualized home health care plans.
  • Administer medications, manage wound care, and perform clinical interventions (including IV therapy) per orders.
  • Monitor and report changes in patient conditions to RN Case Manager or physicians.
  • Educate patients and caregivers on medications, disease management, and self-care strategies.
  • Meticulously document care, assessments, and communication in HCHB.
  • Collaborate with the interdisciplinary healthcare team to ensure care plan compliance.
  • Maintain infection control practices and uphold patient safety standards.
  • Participate in case conferences, continuous improvement initiatives, and required training.

Qualifications:

  • Active, unrestricted LPN license in state of practice.
  • Minimum 1 year of home health, hospice, or similar community-based care experience.
  • Proficiency in Home Care Home Base (HCHB).
  • Valid drivers license, reliable insured vehicle, and willingness to travel within assigned territory.
  • Current BLS (Basic Life Support) certification.
  • Strong time management, clinical judgment, and communication skills.
  • Ability to work independently and manage a varying caseload.

Preferred:

  • Medicare-certified home health agency experience.
  • Strong knowledge of home health documentation and compliance.
  • Excellent interpersonal skills and dedication to patient-centered care.
