Deliver skilled, patient-centered care to geriatric patients in a dynamic home health environment. As an experienced Licensed Practical Nurse, youll make a meaningful impact supporting individuals in their homes, collaborating closely with an interdisciplinary healthcare team. This is a contract role tailored for an LPN who values autonomy, efficiency, and quality care.
Schedule/Hours:
- Contract assignment; specifics will be discussed during the interview process
Key Responsibilities:
- Provide skilled nursing visits to patients in accordance with established care plans
- Monitor and assess patient progress, making recommendations for care plan updates as necessary
- Perform medication administration and treatment within state regulations
- Educate patients and family members regarding disease management and prevention
- Maintain precise and confidential clinical documentation using HomeCare HomeBase (HCHB) EMR
- Collaborate with registered nurses, therapists, and other healthcare professionals to ensure holistic patient care
Desired Qualifications:
- Active and unencumbered LPN license
- At least one year of recent home health nursing experience
- Expertise with HomeCare HomeBase (HCHB) EMR system
- Thorough clinical assessment and documentation competency
- Dependable transportation for patient home visits
- Ability to independently prioritize and manage a caseload
