Job Description

Deliver impactful nursing services in a home health environment, focusing on geriatric patient care. This contract position offers the opportunity to work independently, utilizing your clinical and technical skills to support patient wellness in their homes. Candidates experienced in home health and skilled in HomeCare HomeBase (HCHB) EMR will thrive in this patient-centered care model.

Responsibilities include:

  • Provide skilled nursing care to patients in their homes as outlined in individualized care plans
  • Monitor patient conditions and document progress, updating care plans when necessary
  • Administer medications and perform treatments in line with state regulations
  • Offer education to patients and families on disease prevention and managing care at home
  • Accurately record all clinical documentation in HomeCare HomeBase (HCHB) EMR
  • Collaborate with RNs, therapists, and other healthcare professionals to ensure well-rounded patient care

Schedule/Hours:

  • Contract role with potential for extension

Desired qualifications and experience:

  • Current, active LPN license
  • At least one year of recent home health experience
  • Proficient with HomeCare HomeBase (HCHB) EMR system
  • Strong clinical assessment and documentation skills
  • Reliable vehicle for patient home visits
  • Self-motivated with excellent organization for caseload management
