Provide compassionate, patient-focused care as a Home Health Licensed Practical Nurse (LPN) delivering skilled nursing services directly in patients homes. This full-time contract opportunity offers a dynamic role managing a caseload of community-based patients within an assigned territory. The position emphasizes clinical excellence, effective patient education, and thorough documentation to support patient independence and recovery.

Key Responsibilities:

• Perform skilled nursing visits including assessments, medication administration, wound care, and IV therapy as ordered
• Monitor patient conditions, promptly communicating updates to the RN Case Manager or physician
• Educate patients and caregivers on disease management, medication adherence, and self-care techniques
• Document all nursing care and communications accurately using Home Care Home Base (HCHB) software
• Collaborate closely with the interdisciplinary care team to ensure care plan compliance and quality outcomes
• Maintain infection control standards and promote patient safety during all visits
• Participate in case conferences, ongoing training, and quality improvement initiatives

Qualifications:

• Active and unrestricted LPN license in the state of practice
• Minimum of 1 year experience in home health, hospice, or community-based nursing
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• Valid drivers license, reliable vehicle, and current insurance
• Current BLS (Basic Life Support) certification
• Strong clinical judgment, excellent time management, and communication skills
• Ability to independently manage a diverse caseload in a field-based setting

Preferred Experience:

• Familiarity with Medicare-certified home health agencies
• Solid understanding of home health compliance and documentation standards
• Demonstrated interpersonal skills and a patient-centered care approach

Schedule & Location:

• Monday to Friday, 8:00 AM 5:00 PM (with occasional flexibility based on patient needs)
• 2532 points weekly (roughly 35 patient visits/day)
• Work performed in homes within the designated community territory
• Reliable transportation required; mileage reimbursement typically provided
