Job Description
Provide expert home health nursing as a Licensed Practical Nurse (LPN) delivering compassionate, patient-centered care within the community. This role requires managing a caseload of patients in their homes, promoting recovery and independence through skilled nursing assessments, treatments, and education. Utilize your LPN expertise and proficiency with Home Care Home Base (HCHB) to ensure accurate documentation and compliance with regulatory standards.
Key Responsibilities:
- Conduct skilled nursing visits including assessments, medication administration, wound care, and IV therapy as ordered
- Monitor and report changes in patient conditions promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medications, and self-care techniques
- Document all care and communication accurately using HCHB
- Collaborate with interdisciplinary teams to coordinate and adjust care plans
- Maintain infection control and patient safety standards during all visits
- Participate in case conferences, quality initiatives, and required training sessions
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year home health, hospice, or community-based care experience
- Proficiency in Home Care Home Base (HCHB) software
- Valid drivers license, reliable transportation, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a dynamic caseload in a field-based setting
Preferred:
- Experience with Medicare-certified home health agencies
- Solid knowledge of home health compliance and documentation standards
- Excellent interpersonal skills and dedication to patient-centered care
Position Details:
- Contract role, full-time
- Schedule: Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility
- Caseload: Approximately 35 visits per day (2532 points weekly)
- Mileage reimbursement typically provided
