Deliver high-quality, compassionate nursing care directly to patients in their homes as a skilled Licensed Practical Nurse (LPN) in a home health setting. In this contract role, youll manage a dynamic caseload, partnering closely with patients, families, and the interdisciplinary healthcare team to foster patient recovery, independence, and well-being. This position requires expertise in Home Care Home Base (HCHB), a strong understanding of home health regulations, and the ability to work autonomously in a field-based environment.
Schedule & Work Details:
- Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility based on patient needs)
- Field-based role with a workload of 2532 points per week (generally 35 patient visits per day)
- Reliable transportation required for travel within assigned patient territory
Key Responsibilities:
- Deliver skilled nursing care and assessments in patients homes
- Administer medications, perform wound care, and manage IV therapy
- Monitor patient conditions and promptly communicate changes to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication, and self-care
- Maintain accurate and thorough documentation utilizing Home Care Home Base (HCHB)
- Ensure safe care practices and infection control standards at all times
- Collaborate with the care team, participating in case conferences and quality improvement initiatives
Required Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or related community care experience
- Strong proficiency with Home Care Home Base (HCHB) documentation
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Excellent communication, clinical judgment, and organizational skills
Preferred Experience:
- Experience within Medicare-certified home health agencies
- Thorough knowledge of home health compliance and documentation requirements
