Job Description

Deliver compassionate, patient-focused care as a Home Health Licensed Practical Nurse (LPN) serving individuals in their homes. This full-time contract role offers the opportunity to manage a dynamic caseload while utilizing your clinical expertise and knowledge of home health compliance standards.

Location:
Based in Mesa, AZ, this position involves providing skilled nursing care to patients within an assigned territory. Responsibilities:

  • Conduct skilled nursing visits to assess and implement individualized care plans in patients homes
  • Administer medications, perform wound care, manage IV therapy, and other clinical procedures as ordered
  • Monitor patient conditions closely and communicate changes promptly to the RN Case Manager or physicians
  • Educate patients and caregivers on disease management, medications, and self-care techniques
  • Thoroughly document clinical assessments and care activities using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary teams to ensure coordinated, compliant patient care
  • Maintain infection control and safety standards throughout patient interactions
  • Participate in quality improvement initiatives and required training

Qualifications and Desired Experience:

  • Active, unrestricted LPN license in the practicing state
  • Minimum 1 year of home health, hospice, or community-based nursing experience
  • Proficiency with Home Care Home Base (HCHB) system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Strong clinical judgment, excellent communication, and time management skills
  • Ability to work independently and manage a caseload efficiently
  • Preferred: Experience with Medicare-certified home health agencies and thorough understanding of home health compliance

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility as patient needs arise)
  • Approximately 35 patient visits per day, totaling 2532 points per week
