Job Description

Deliver compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) supporting patients in their residences. This full-time contract position involves managing a caseload of home health patients, ensuring compliance with regulatory standards, and collaborating closely with an interdisciplinary team to promote patient independence and recovery.

Schedule:

  • Monday Friday
  • 8:00 AM 5:00 PM (with occasional flexibility for patient needs)

Key Responsibilities:

  • Conduct skilled nursing assessments and implement individualized care plans in patients' homes.
  • Administer medications, perform wound care, and manage clinical interventions such as IV therapy.
  • Monitor patient conditions and promptly communicate changes to RN Case Managers or physicians.
  • Educate patients and caregivers on disease management, medication use, and self-care techniques.
  • Document all care and communications accurately using Home Care Home Base (HCHB) software.
  • Collaborate with the healthcare team to ensure coordinated care and compliance with care plans.
  • Maintain infection control protocols and prioritize patient safety.
  • Participate in case conferences, quality improvement initiatives, and mandatory training.

Qualifications:

  • Active, unrestricted LPN license valid in the state of practice.
  • Minimum of 1-year experience in home health, hospice, or community-based nursing.
  • Proficiency with Home Care Home Base (HCHB) charting system.
  • Valid drivers license, reliable vehicle, and current insurance.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical judgment, communication, and time management skills.
  • Ability to work independently in a field-based role managing a dynamic patient caseload.

Preferred:

  • Experience with Medicare-certified home health agencies.
  • Deep knowledge of home health compliance and documentation.
  • Patient-focused and excellent interpersonal skills.
