Job Description

Required Qualifications:

  • Active LPN license
  • At least one year of recent home health experience
  • Proficiency using HomeCare HomeBase (HCHB) EMR
  • Excellent clinical assessment and documentation abilities
  • Dependable transportation for patient visits
  • Strong self-management skills and ability to work independently

Primary Responsibilities:

  • Deliver skilled nursing care to patients in their homes based on individualized care plans
  • Monitor patient conditions, document progress, and update care plans as necessary
  • Administer medications and perform assigned treatments in accordance with state guidelines
  • Educate patients and family members on care practices, health management, and disease prevention
  • Maintain accurate and timely documentation using HomeCare HomeBase (HCHB) EMR system
  • Collaborate with RNs, therapists, and other healthcare professionals to ensure comprehensive, high-quality patient care
