Deliver exceptional patient-centered care as a Licensed Practical Nurse (LPN) in a home health environment. This contract role is designed for an experienced LPN who is comfortable working independently to support geriatric patients with their healthcare needs. Qualifications:
- Active, unrestricted LPN license
- At least one year of recent experience in home health nursing
- Strong proficiency with HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment and documentation capabilities
- Reliable transportation for patient home visits
- Proven ability to efficiently manage an independent caseload
Responsibilities:
- Deliver skilled nursing care to patients in their homes following established care plans
- Monitor, document, and report patient progress; update care plans accordingly
- Administer medications and perform prescribed treatments in line with state regulations
- Educate patients and their families on disease prevention and effective care management strategies
- Maintain precise records using HomeCare HomeBase (HCHB) EMR system
- Collaborate with RNs, therapists, and other healthcare professionals for comprehensive patient care
