Required Qualifications:
- Active LPN license
- At least one year of recent home health experience
- Proficiency using HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment and documentation abilities
- Dependable transportation for patient visits
- Strong self-management skills and ability to work independently
Primary Responsibilities:
- Deliver skilled nursing care to patients in their homes based on individualized care plans
- Monitor patient conditions, document progress, and update care plans as necessary
- Administer medications and perform assigned treatments in accordance with state guidelines
- Educate patients and family members on care practices, health management, and disease prevention
- Maintain accurate and timely documentation using HomeCare HomeBase (HCHB) EMR system
- Collaborate with RNs, therapists, and other healthcare professionals to ensure comprehensive, high-quality patient care
