As a Home Health Licensed Practical Nurse (LPN), you will make a positive impact, fostering independence, comfort, and safety for individuals on their road to recovery. This position offers an engaging, field-based environment and the opportunity to form meaningful connections with patients while utilizing your clinical and care coordination skills.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Occasional flexibility may be required to accommodate patient needs

Key Qualifications:

• Active and unrestricted LPN license in the state of practice
• At least 1 year of experience in home health, hospice, or similar community-based care setting
• Proficient in Home Care Home Base (HCHB) charting system
• Current BLS (Basic Life Support) certification
• Reliable transportation, valid drivers license, and up-to-date insurance
• Ability to work independently, manage a dynamic caseload, and prioritize effectively

Preferred Experience:

• Background in Medicare-certified home health agencies
• Strong knowledge of home health compliance and documentation standards
• Excellent interpersonal and patient education skills

Primary Responsibilities:

• Deliver skilled nursing visits in patients homes, assessing, planning, and implementing individualized care plans
• Administer medications, perform wound care, and manage IV therapy as needed
• Closely monitor patients and promptly communicate changes to the clinical team
• Educate patients and caregivers on disease management and self-care
• Complete timely and accurate documentation using HCHB
• Collaborate with the interdisciplinary team and participate in quality improvement activities
