Deliver compassionate care where it matters most as a Home Health Licensed Practical Nurse (LPN). This contract opportunity offers you the chance to make a meaningful impact on patients lives, supporting them in the comfort of their own homes. Youll be instrumental in promoting recovery, wellness, and independence while working closely with patients, families, and the interdisciplinary care team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year of home health, hospice, or community-based experience
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable vehicle, and current insurance
- BLS (Basic Life Support) certification
- Strong assessment skills, time management, and communication abilities
- Experience working independently in a field-based setting
Preferred Candidates Will Also Have:
- Background with Medicare-certified home health agencies
- Solid understanding of compliance and documentation standards
- Excellent interpersonal skills and a dedication to patient-centered care
Primary Responsibilities:
- Conduct skilled nursing visits, assessing and implementing individualized care plans
- Administer medications, wound care, and clinical interventions as ordered
- Monitor patient condition changes and report promptly to case management
- Educate patients and caregivers on self-care and disease processes
- Meticulously document all patient interactions and care in Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary team to coordinate effective care
- Adhere to infection control standards, ensuring patient safety at all times
- Actively participate in case conferences and quality improvement activities
