Job Description
Deliver compassionate, patient-focused nursing care in a home health setting as a Licensed Practical Nurse. This full-time contract role provides the opportunity to make a meaningful difference in patients lives, focusing on independence, education, and recovery in the comfort of their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required for patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or similar setting
- Demonstrated proficiency using Home Care Home Base (HCHB)
- Current BLS certification
- Valid drivers license, reliable transportation, and current insurance
- Ability to work independently in the field and manage a dynamic caseload
- Strong clinical judgment, time management, and communication skills
Responsibilities:
- Manage a weekly caseload with 2532 points (typically 35 patient visits per day)
- Deliver skilled nursing visits: assess, plan, implement and evaluate individualized care plans
- Administer medications, provide wound care, manage IV therapy and other clinical interventions
- Monitor and report patient condition changes to RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care
- Document care using HCHB, ensuring compliance with regulatory and organizational standards
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Uphold infection control and patient safety standards at all times
- Participate in ongoing training and quality improvement meetings
Preferred Experience:
- Background working with Medicare-certified home health agencies
- Strong understanding of home health compliance
