Deliver skilled, patient-focused nursing care in home settings as a Home Health Registered Nurse (RN) on a full-time, 13-week contract in the Marietta, GA area. This role emphasizes autonomy, clinical proficiency, and compassionate communication while supporting patients in their homes.

Key Responsibilities:

• Perform in-depth OASIS assessments and formulate personalized care plans.
• Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes, and communicate promptly with physicians and the healthcare team.
• Educate patients and families on treatment plans, disease management, medications, and preventive care.
• Accurately document care and assessments using the Home Care Home Base (HCHB) electronic system.
• Collaborate with interdisciplinary team members to coordinate comprehensive care.
• Attend case conferences, staff meetings, and participate in continuing education.
• Ensure compliance with all regulatory and agency standards.

Qualifications and Experience:

• Active RN license in good standing within the state.
• Minimum one year recent home health experience; field experience highly preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments is required.
• Strong clinical judgment, critical thinking, and communication skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Ability to work independently with effective time management and organizational skills.
• BSN preferred.
• Familiarity with managing 2532 points weekly and Medicare/home health compliance is a plus.

Schedule and Location:

• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload typically includes 35 patient visits daily (2532 points weekly).
• Role requires daily travel to patient homes within the assigned territory; mileage reimbursement provided.
