Job Description
Deliver quality, patient-centered care as a Home Health Licensed Practical Nurse (LPN) and make a meaningful difference in your community. This contract position provides an opportunity to support recovery and independence for individuals receiving in-home health services, while collaborating closely with an interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility for patient needs)
- Typical workload: 2532 points per week, equating to approximately 35 patient visits daily
Key Responsibilities:
- Perform skilled nursing assessments and implement individualized care plans in patients homes
- Administer medications, provide wound care, and manage IV therapy and clinical interventions
- Monitor patient health status, document findings, and report changes to supervising clinicians
- Educate patients and caregivers on disease management, medication safety, and self-care practices
- Ensure accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with the healthcare team to coordinate and optimize patient care
- Maintain infection prevention protocols, safety standards, and uphold regulatory compliance
- Participate in case conferences, quality assurance, and continued education activities
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year of home health, hospice, or community-based care experience
- Proficiency in Home Care Home Base (HCHB) required
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable transportation, and current auto insurance
- Ability to work independently, prioritize effectively, and demonstrate strong communication skills
Preferred Experience:
- Background in Medicare-certified home health agencies
- Advanced knowledge of compliance guidelines and documentation requirements
- Excellent interpersonal skills and a compassionate, patient-focused approach
