Job Description
Deliver impactful home-based healthcare and make a difference as a Licensed Practical Nurse. This contract role is ideal for an experienced caregiver who thrives in an independent, field-based setting and has a strong background in home health nursing.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least one year of experience in home health, hospice, or community-based care
- Skilled in using Home Care Home Base (HCHB) for charting and compliance
- Current BLS (Basic Life Support) certification
- Valid driver's license and reliable vehicle with current insurance
- Strong clinical judgment, time management, and communication abilities
- Ability to work both independently and collaboratively
- Preferred: Experience with Medicare-certified home health agencies, excellent knowledge of home health documentation standards, and a patient-centered care approach
Core Responsibilities:
- Conduct skilled nursing visits and implement individualized patient care plans in home settings
- Administer medications, provide wound care, manage IV therapy, and perform clinical interventions as ordered
- Assess and monitor patient condition changes, reporting to the RN Case Manager or physician as necessary
- Educate patients and caregivers on medical care, medication, and self-management techniques
- Maintain accurate documentation through Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members to coordinate and optimize patient care
- Uphold infection control and patient safety standards at all times
- Participate in case conferences, required training, and continuous quality improvement initiatives
