Job Description

Deliver impactful home-based healthcare and make a difference as a Licensed Practical Nurse. This contract role is ideal for an experienced caregiver who thrives in an independent, field-based setting and has a strong background in home health nursing.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional flexibility for patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least one year of experience in home health, hospice, or community-based care
  • Skilled in using Home Care Home Base (HCHB) for charting and compliance
  • Current BLS (Basic Life Support) certification
  • Valid driver's license and reliable vehicle with current insurance
  • Strong clinical judgment, time management, and communication abilities
  • Ability to work both independently and collaboratively
  • Preferred: Experience with Medicare-certified home health agencies, excellent knowledge of home health documentation standards, and a patient-centered care approach

Core Responsibilities:

  • Conduct skilled nursing visits and implement individualized patient care plans in home settings
  • Administer medications, provide wound care, manage IV therapy, and perform clinical interventions as ordered
  • Assess and monitor patient condition changes, reporting to the RN Case Manager or physician as necessary
  • Educate patients and caregivers on medical care, medication, and self-management techniques
  • Maintain accurate documentation through Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary team members to coordinate and optimize patient care
  • Uphold infection control and patient safety standards at all times
  • Participate in case conferences, required training, and continuous quality improvement initiatives
