Job Description
Provide skilled, compassionate nursing care as a Home Health Licensed Practical Nurse (LPN) delivering services directly in patients' homes within the Louisville, KY area. This full-time contract role offers the opportunity to promote patient independence, educate caregivers, and support recovery while managing a caseload in a community-based setting.
Responsibilities include conducting skilled nursing visits to assess patient needs, administering medications and wound care, managing IV therapies, and monitoring patient conditions. You will document all care and communications accurately using Home Care Home Base (HCHB), collaborate with interdisciplinary teams, and ensure compliance with care plans and infection control standards. Participation in case conferences and ongoing training is also required.
Qualifications:
- Active, unrestricted LPN license valid in Kentucky
- Minimum of 1 year of experience in home health, hospice, or similar community care
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable insured vehicle, and ability to travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently and manage a dynamic caseload
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Thorough understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-centered approach
Schedule & Workload:
- Monday through Friday, 8:00 AM to 5:00 PM with occasional flexibility
- Approximately 35 patient visits per day (2532 points weekly)
