Job Description
Deliver high-quality nursing care directly to patients in their homes as a dedicated Home Health Licensed Practical Nurse (LPN). This contract, full-time position offers a meaningful opportunity to serve your community, supporting patients independence and recovery in a one-on-one environment. You will work autonomously in the field, develop patient-centered care plans, and collaborate with an interdisciplinary team to optimize patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (flexibility for patient needs)
- Workload of approximately 2532 points per week (about 35 patient visits daily)
- Field-based, serving patients within an assigned local territory
Key Responsibilities:
- Complete comprehensive and skilled nursing visits for home health patients
- Administer medications, wound care, and IV therapy as prescribed
- Monitor and document changes in patient conditions, promptly reporting to the RN Case Manager or physician
- Educate patients and caregivers about disease management, medications, and self-care
- Maintain accurate and timely charting using Home Care Home Base (HCHB)
- Collaborate with the care team to ensure effective care coordination and plan adherence
- Uphold infection control and patient safety protocols
- Participate in case conferences and quality improvement initiatives
Required Qualifications:
- Active, unrestricted LPN license in Colorado
- Minimum 1 year of home health, hospice, or similar community-based care experience
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable vehicle, and current auto insurance
- Current BLS certification
- Strong clinical judgment, communication, and time management skills
- Ability to self-direct in a field-based setting
Preferred:
- Previous experience with Medicare-certified home health agencies
- Strong knowledge of home health compliance and documentation standards
