Deliver comprehensive nursing care as a Home Health Licensed Practical Nurse (LPN) in a rewarding field-based role. This position offers the opportunity to work directly with patients in their residences throughout an assigned territory, promoting patient independence, education, and recovery. Focus on providing tailored interventions while maintaining accurate documentation aligned with regulatory and organizational standards.

Schedule/Hours:
- Full-time contract role
- Monday Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs
- Manage 2532 points weekly, approximately 35 patient visits daily
Key Responsibilities:
- Conduct skilled nursing assessments, develop, and implement individualized care plans in patients' homes
- Administer medications, perform wound care, and manage clinical interventions including IV therapy
- Monitor patient conditions, report changes promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Document all care activities using Home Care Home Base (HCHB) system with precision
- Collaborate with interdisciplinary teams ensuring care coordination and compliance
- Uphold infection control practices and patient safety standards
- Participate in case conferences and quality improvement initiatives
Qualifications & Desired Experience:
- Active, unrestricted LPN license in the state of Arkansas
- Minimum one year of home health, hospice, or community-based nursing experience
- Proficient with Home Care Home Base (HCHB) documentation system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Strong clinical judgment, time management, and effective communication skills
- Ability to work independently managing a dynamic caseload
Preferred: Prior Medicare-certified home health experience and thorough understanding of home health compliance
