Job Description
Join a dedicated healthcare team delivering compassionate and skilled care to patients in their homes as a Licensed Practical Nurse (LPN). This contract position focuses on providing clinical excellence and patient education within home health, ensuring the wellbeing and autonomy of each individual served.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of recent experience in home health, hospice, or community-based care
- Proficiency in Home Care Home Base (HCHB) charting system
- Current BLS (Basic Life Support) certification
- Valid drivers license, insurance, and reliable transportation
- Strong clinical skills, time management, and effective communication
- Ability to work independently in the field and manage a dynamic caseload
Preferred Experience:
- Prior work with Medicare-certified home health agencies
- Solid understanding of home health compliance and documentation standards
- Excellent interpersonal and patient-centered approach to care
Primary Responsibilities:
- Conduct skilled nursing assessments and develop individualized care plans for home-based patients
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy
- Monitor patient conditions, promptly communicating any changes to the RN Case Manager or physician
- Provide in-depth education to patients and caregivers on managing health conditions and self-care
- Maintain accurate, timely documentation using HCHB and ensure all care aligns with established protocols
- Work collaboratively with the broader medical team for coordinated patient care
- Uphold infection control and safety practices throughout care delivery
- Engage in ongoing professional training and quality improvement initiatives
