Job Description

Join a dedicated healthcare team delivering compassionate and skilled care to patients in their homes as a Licensed Practical Nurse (LPN). This contract position focuses on providing clinical excellence and patient education within home health, ensuring the wellbeing and autonomy of each individual served.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to accommodate patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of recent experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB) charting system
  • Current BLS (Basic Life Support) certification
  • Valid drivers license, insurance, and reliable transportation
  • Strong clinical skills, time management, and effective communication
  • Ability to work independently in the field and manage a dynamic caseload

Preferred Experience:

  • Prior work with Medicare-certified home health agencies
  • Solid understanding of home health compliance and documentation standards
  • Excellent interpersonal and patient-centered approach to care

Primary Responsibilities:

  • Conduct skilled nursing assessments and develop individualized care plans for home-based patients
  • Administer medications, perform wound care, and manage clinical interventions such as IV therapy
  • Monitor patient conditions, promptly communicating any changes to the RN Case Manager or physician
  • Provide in-depth education to patients and caregivers on managing health conditions and self-care
  • Maintain accurate, timely documentation using HCHB and ensure all care aligns with established protocols
  • Work collaboratively with the broader medical team for coordinated patient care
  • Uphold infection control and safety practices throughout care delivery
  • Engage in ongoing professional training and quality improvement initiatives
