This contract opportunity is designed for licensed practical nurses who excel at providing individualized healthcare services in patients homes, supporting positive outcomes while working independently within a multidisciplinary team.
Schedule/Hours:

Your background in Home Health and LPN expertise will help patients recover and thrive at home. This role requires strong communication and clinical skills, attention to detail, and an empathetic approach to patient and family education.
Qualifications:
- Current LPN license in the state of Georgia
- Minimum one year of recent experience as an LPN in a Home Health setting preferred
- Valid BLS (Basic Life Support) certification
- Proficient with electronic documentation and care planning
- Ability to work effectively with both clinical and non-clinical staff
- Exceptional organizational skills and self-motivation
- Reliable transportation and valid drivers license
Responsibilities:
- Perform skilled nursing care and treatments in patients homes in accordance with individualized care plans
- Accurately document patient status, interventions, and progress using electronic health records
- Educate patients and families on disease processes, medications, and self-care techniques
- Collaborate with RNs, therapists, physicians, and support staff to ensure coordinated care
- Monitor and report any changes in patient condition promptly
- Adhere to infection control protocols and safety standards during all patient interactions
