Utilize your clinical expertise and familiarity with HomeCare HomeBase (HCHB) EMR to make a meaningful contribution to each patient's health and quality of life.
Essential Qualifications:
- Active LPN license in good standing
- At least one year of recent experience in home health
- Proficiency with HomeCare HomeBase (HCHB) electronic medical records
- Excellent clinical assessment and documentation abilities
- Strong organizational skills and the ability to effectively manage a caseload
- Reliable transportation for patient home visits
- Confident working both independently and as part of an interdisciplinary team
Key Responsibilities:
- Deliver skilled nursing care in patients homes based on individualized care plans
- Monitor, document, and communicate patient progress, updating plans as appropriate
- Administer medications and perform treatments within state and agency guidelines
- Educate patients and families on condition management, medications, and health promotion
- Maintain precise and timely records in the HCHB system
- Coordinate closely with registered nurses, therapists, and other care professionals to ensure optimal patient outcomes
