Schedule/Hours:

  • Contract-based assignment, potential for extension

Qualifications:

  • Active and unrestricted LPN license
  • At least one year of recent experience in home health care
  • Skilled in using HomeCare HomeBase (HCHB) EMR for clinical documentation
  • Solid clinical assessment abilities and attention to detail
  • Reliable vehicle and willingness to travel for patient visits
  • Strong communication skills and the ability to manage a caseload effectively

Primary Responsibilities:

  • Deliver high-quality nursing care in patients homes following individualized care plans
  • Monitor and record patient progress, making adjustments to care as necessary
  • Administer medications and perform treatments in alignment with state regulations
  • Educate patients and their families on disease prevention, home-based care, and health management
  • Maintain accurate and timely records using HCHB EMR
  • Collaborate with RNs, therapists, and other health professionals to coordinate comprehensive care
