Job Description

An opportunity exists for a caring and experienced Licensed Practical Nurse (LPN) to deliver skilled home health services to patients in a supportive field-based role. This position ensures patient-centered nursing care in the comfort of individuals homes, placing an emphasis on safety, independence, and education.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to meet specific patient needs

Key Responsibilities:

  • Provide skilled nursing visits to develop and implement individualized care plans
  • Administer medications, perform wound care, provide IV therapy, and other clinical interventions as ordered
  • Assess changes in patient condition and promptly communicate with interdisciplinary team members, including the RN Case Manager or physician
  • Educate patients and caregivers regarding disease processes, medication regimens, and health management strategies
  • Document assessments, care provided, and communications accurately in Home Care Home Base (HCHB)
  • Collaborate closely with peers to coordinate comprehensive care and ensure plan adherence
  • Uphold infection control measures and prioritize patient safety at every visit
  • Attend case conferences, participate in quality improvement, and complete required trainings

Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • At least 1 year of home health, hospice, or similar community-based care experience
  • Demonstrated experience and proficiency with Home Care Home Base (HCHB)
  • Valid drivers license, reliable transportation, and current vehicle insurance
  • BLS (Basic Life Support) certification
  • Excellent clinical decision-making, communication, and organizational skills
  • Ability to manage a dynamic caseload independently

Preferred Experience:

  • Service with Medicare-certified home health agencies
  • Strong grasp of home health compliance and documentation requirements
  • Compassionate, patient-first approach and outstanding interpersonal skills
